Medical Information Form

Certain conditions require that a Medical Information Form is completed, signed by your doctor and sent by Fax to: (01) 8866868.

Expectant Mother Travel Advice Form

Depending on travel routes and stage of pregnacy, a passenger may need to complete an Expectant Mother Travel Advice Form.

Unaccompanied Young Passenger Form

Young passengers 12 and older may travel unaccompanied with a form of indemnity. A Form of Indemnity must be completed at each origin airport.

Please note that for Aer Lingus-JetBlue Airways connecting flights, unaccompanied minors under the age of 14 will not be accepted by either carrier. Unaccompanied minors under the age of 14 departing Italy will also not be accepted for travel.
